New patient information

We truly look forward to taking care of you for your physical therapy needs.  Please bring all of your medical records and pertinent information to your appointment; including your X-ray(s), MRI (physical CD), prescription from your referring physician, and operative note (if applicable).  Prior to your visit, please contact your insurance company to verify your physical therapy benefits.  We do not bill insurance directly, but will provide you with a medical superbill to submit to your insurance company.
